Role Summary
The Advanced Nurse Practitioner (ANP) will provide high-quality, evidence-based clinical care to patients in a primary care environment. Working autonomously, the postholder will assess, diagnose, and manage patients presenting with a range of health needs, ensuring timely access to care and supporting the wider multidisciplinary team.
Main Responsibilities
* Independently assess, diagnose, and manage patients presenting with undifferentiated and undiagnosed conditions.
* Request, interpret, and act on results of investigations and tests.
* Prescribe medication safely and appropriately as an Independent Prescriber.
* Conduct medication reviews and provide lifestyle and health education advice.
* Refer patients to other healthcare professionals or specialist services as required.
* Support chronic disease management and preventative care initiatives.
* Maintain accurate and contemporaneous clinical records in accordance with professional and organisational standards.
* Contribute to clinical governance, audit, and service development within the practice.
* Work collaboratively with GPs, nurses, and administrative staff to ensure seamless patient care.
Qualifications & Requirements
Essential
* Registered Nurse with the Nursing and Midwifery Council (NMC).
* Advanced Practice qualification (Master’s level or equivalent).
* Independent Nurse Prescriber qualification.
* Proven experience working in primary care or a similar clinical setting.
Desirable
* Experience managing long-term conditions and acute presentations in a general practice setting.
* Knowledge of QOF, clinical templates, and electronic health record systems.
* Commitment to continuing professional development and clinical leadership.
