What the Job Involves As a Leaving Care Worker, you will serve as the primary point of contact and support for young people transitioning out of care. You will coordinate resources from various agencies to ensure that young people transition to adulthood safely and healthily. This role involves direct work with young people and their families, assisting them in achieving their individual plans and goals, and encouraging their participation in decision-making processes. You will also support young people at risk for negative outcomes and manage crises through a duty service. Your responsibilities will include managing a caseload, preparing reports, maintaining records, and ensuring compliance with relevant policies and procedures.
