Care Manager Job Description

To promote and manage a needs led service for adults who have a learning disability and or Autism. You will lead a care and support team aspiring to deliver outstanding care by adopting a person centred approach that fosters self-awareness, personal growth and gives each person the strongest voice with regards to decision making and lifestyle choices so that they can feel safe and supported at home and in the local community.

The right candidate will be able to demonstrate their skills in motivating, managing and developing staff, alongside a clear understanding of leadership responsibilities in social care and excellent communication and interpersonal skills. They will be passionate about delivering excellent quality, able to manage a varied workload effectively, and instil a culture of continuous improvement through dialogue with service users, their families, advocates, and others.

Experience of supporting people with high and complex support needs, and skills around positive behaviour support would also be an advantage. You will have good IT skills to complete daily administrative and management tasks including

(but not limited to) use of Excel Spreadsheets and Emails..

Main responsibilities, tasks and duties

Leadership

1. Promote uphold and reinforce the vision and aims of the service and ensure all expected outcomes are achieved

2. Provide leadership to the team, model best practice and be confident in managing performance

3. Promote and maintain a culture and environment that is safe, comfortable and promotes best practice

4. Ensure all staff adhere to all relevant legislation and policies and procedures

5. Manage rotas on a weekly basis ensuring Working Time Directive and other Employee related law and guidance is followed

6. Plan ahead for annual leave and pressures on services to ensure continuity of care

7. Ensure service users rights and freedoms are safeguarded at all times

8. Supervise the shift, ensuring staff give support that follows individual's support plans and risk management protocols.

9. Provide effective supervision to the Care and Support Workers on your team.

10. Provide On-Call cover as required, working within on call guidance.

11. To identify the training needs of your team and ensure effective induction, training and professional development of all new staff.

12. Motivate, support and mentor your staff team to enable them to succeed and meet the expectations of their role

13. Ensure that risk assessments and care plans are updated regularly and any risks are managed effectively

14. Ensure all repairs and maintenance issues are reported and followed up on

15. Ensure incidents and accidents are reported and scrutinised

16. Ensure staff complete all necessary paperwork to an acceptable standard and sign off.

17. Effectively monitor the service delivery including reviewing daily records and logs to identify areas of concern, patterns and trends

Support

1. Lead your team to empower individuals to make choices, enjoy opportunities for personal development and achieve their aspirations and personal goals.

2. Ensure you and your team are supporting Service Users with regards to decision making and lifestyle choices enabling them to take control of their own lives, increasing independence.

3. Ensure you and your team carry out all personal care tasks in line with the needs, wishes and preferences of the individual, following the care plan and with respect and dignity.

4. Ensure you and your team support Service Users to take an active role in the community by accessing local facilities and wider community resources in the spirit of social inclusion.

5. Lead on and support the development of person-centred planning, ensuring each individual's plan has meaningful and achievable goals.

6. Lead your team to support Service Users to develop and expand social networks and friendships.

7. Lead on supporting your team through the service user transition process.

8. Coordinate and develop the support to the individual including (but not restricted to) Health Action Plans and Person-Centred Plans, Hospital Passports, Communication Plans, Positive Behaviour Support Plans.

9. Prepare in advance for, contribute to / attend reviews and multi-disciplinary meetings and ensure any actions are followed up.

10. Work in partnership to promote healthy lifestyles by liaising with local health care professionals as part of our care and support planning activities.

11. Work in partnership with senior managers and other professionals to maximise quality of life for individuals.

12. Develop positive relationships with family and support staff.

13. Work in line with the company's Adult Protection and Prevention of Abuse Policy and Whistle Blowing Policy to ensure that people are always kept safe.

14. Work within equal opportunities policy and procedure.

15. Ensure risk assessments are actioned as necessary, in line with policies and procedures.

16. To be flexible and responsive always to meet the changing needs of the service and the service users.

17. Organise and support service user holidays in line with the policy.

18. Build supportive relationships and work effectively with Service Users who may present challenging and difficult behaviour.

Governance

1. To assist the Company to implement the monitoring and quality control processes under the Company's Quality Monitoring initiatives.

2. To familiarise with Company's Audit tool in order to conduct and implement quality assurance systems in a timely manner.

3. To attend company management meetings as required

4. Work in a way that meets the statutory requirements of employees under Health and Safety at Work and other Employee related legislation such as Working Time Directive, GDPR for example

5. Deal with complaints and concerns that are raised and respond to these appropriately for mutual satisfaction or escalate the concerns to Operations Manager

6. At all times work within regulatory guidelines, current legislation and best practice guidance.

7. To sign off all repairs works as satisfactory and complete before invoices are submitted for payment

8. To develop professional relationships with families built on integrity, honesty and trust that enable you and your team to safeguard the best interests of you clients at all times

9. Attend training, and ensure your team are available to, on a regular basis to ensure that all mandatory and personal training and development needs are met in line with company requirements.

10. To behave and conduct yourself in a manner that reflects positively on the company and is in keeping with your role as a professional at all times.

11. To work closely with other members of the team for the ultimate benefit of the people who reside at the property.

Finance

1. Support Service Users to manage their resources and finances.

2. Deliver training for new staff and refreshers for existing team members

3. Ensure all staff know and understand their legal obligations and responsibilities to safeguard clients finances

4. To ensure that all receipts, change and any accompanying paperwork are intact before the end of the shift.

5. To give practical support to individuals in line with their care plan and policy.

6. Ensure Petty Cash and people's monies are checked, balanced and recorded at the end of every shift.

7. Ensure Financial discrepancies are investigated and reported to senior management.

Administration

1. Ensure work is recorded accurately and appropriately in compliance with the company requirements and that records made and personal information used are in compliance with the GDPR and the standards of information governance.
