Overview
An excellent full-time Band 5 Nurse position within the SACT Outreach Team. The SACT Outreach Team delivers treatments for patients closer to home, providing care away from main sites and covering additional duties at Oak Road Treatment Centre when required. Sites include St Ann's Little Hulton, New Mills, Dr Kershaw's Hospice, Bury Hospice, St Luke's Hospice & Tameside MacMillan Unit. The role involves coordinating outreach clinics, developing clinical skills within SACT, teaching others, and managing staff and their development and attendance. Responsibilities
* Provide a supportive role within the SACT outreach sites for nursing, medical and clerical staff and cover between Chemotherapy Services Units and other SACT outreach sites.
* Undertake nursing care of patients, ensuring privacy and dignity while addressing physical and psychological needs.
* Provide clear and accurate handover at changes of duties and document nursing interventions accurately.
* Participate in nursing procedures appropriate to the department.
* Support and assist induction/training of junior staff and Health Care Assistants, acting as mentor to designated staff.
* Welcome patients to the site/unit and support routine operations.
* Act as coordinator to ensure safe and timely management of patients.
* Liaise with medical and non-medical personnel and community organisations.
* Rotate/work on ORTC SACT unit if required.
* Highlight areas for improvement within the department and undertake relevant projects.
* Provide support for other nurses on site.
* Assist in provision of meals/refreshments for patients while meeting nutritional needs.
* Assist in general housekeeping duties across all areas.
* Help maintain a safe environment and report faults/risks to the nurse in charge.
* Liaise with the nurse responsible for stores/stationery and assist in stock maintenance.
* Handle patients’ personal possessions and ensure cash/valuables are kept securely.
* Aid the safe storage of medicines and the return of unused SACT to pharmacy.
* Assist medical staff with procedures.

* Maintain confidentiality regarding patient information at all times.
* Adhere to all policies including sickness/absence, confidentiality, uniform and Health and Safety at Work Act requirements.
* Report and record accidents and complaints according to policy.
* Be aware of limitations and utilise the preceptor system for issues that arise.
* Respect team members and support the SACT Day Services philosophy of mutual support.
