Advanced Nurse/Clinical Practitioner Primary Care

Southwell Medical Centre are looking for an experienced Advanced Nurse Practitioner/Advanced Clinical Practitioner to work up to full time.

We are looking for friendly, conscientious team members to compliment our current multi-disciplinary team in our delivery of primary health care to our patient population.


Main duties of the job

Responsibilities include:

Develop, implement, and embed health and wellbeing programs tailored to community needs

Perform on-call sessions with the on-call GP to triage patients effectively

Manage acute and chronic conditions, providing expert advice and creating specialized treatment plans for chronic disease patients.

Identify and support at-risk patients, mitigating long-term health complications.

Deliver advanced care in line with clinical evidence, NICE guidelines, and NSF standards.

Request, collect, and interpret pathology results, ensuring comprehensive patient assessment and treatment.

Offer consultations, adjust care plans, and refer patients to secondary or specialist care as needed.

Recognise, assess, and signpost patients presenting with mental health concerns.

Act as an independent or supplementary prescriber in line with professional guidelines.

Support clinical emergencies and handle safeguarding matters per national and local policies.

Understand and manage cases involving substance abuse or addictive behaviors.

Deliver opportunistic health advice during patient interactions.

Provide care to housebound patients as required.

This role also involves contributing to practice targets (e.g., QOF), delegating tasks appropriately, and liaising with external services to ensure holistic patient support.


About us

Southwell Medical Centre is a semi-rural practice in the heart of Nottinghamshire, serving a patient population of circa 12300 patients.

We have a large multi-disciplinary clinical team including permanently employed staff as well as ARRS staff employed by our PCN.

We are a really friendly team, looking to expand to ease the pressure of the demand for routine and acute appointments. We believe in prioritising patient care at the same time as balancing the needs and welfare of our team.

We offer enrolment into the NHS Pension Scheme, have on-site parking and also offer an employee assistance programme.


Job responsibilities

Develop,implement and embed health promotion and wellbeing programmes

Undertakeon-call sessions alongside the on-call GP to triage patients as part of a totaltriage system

Manage patientspresenting with a range of acute and chronic medical conditions, providingsubject matter expert advice

Implement andevaluate individual specialised treatment plans for chronic disease patients

Identify,manage and support patients at risk of developing long-term conditions,preventing adverse effects to the patients health

Provideadvanced, specialist care to patients as required in accordance with clinicalbased evidence, NICE and the NSF

Undertake thecollection of pathological specimens

Requestpathology services as necessary

Process andinterpret pathology and other test results as requested and also providingbuddy assistance to other clinicians as required

Provide chronicdisease consultations, delivering patient care as necessary, referring patientsto secondary/specialist care as required

Maintainaccurate clinical records in conjunction with extant legislation

Ensure SNOMEDCT codes are used effectively

Prioritisehealth issues and intervene appropriately

Support theteam in dealing with clinical emergencies

Recognise,assess and refer/signpost patients presenting with mental health needs whereclinically indicated

Be an extendedand supplementary prescriber, adhering to extant guidance

Contribute topractice targets (QOF, etc.), complying with local and regional guidance

Liaise withexternal services/agencies to ensure the patient is supported appropriately(vulnerable patients, etc.)

Delegateclinical responsibilities appropriately (ensuring safe practice and that thetask is within the scope of practice of the individual)

Support theclinical team with all safeguarding matters in accordance with local andnational policies

Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately

Deliveropportunistic health promotion where appropriate

Undertaking homevisits to housebound patients where necessary


Person Specification


Qualifications

* Registered with the Nursing and Midwifery Council
* Masters degree qualification in line with RCN Credentialing for Advanced Level of Nursing Practice - Post December 2020
* Post graduate diploma or degree for Advanced Practice Qualification - prior to December 2020
* NMC revalidation requirements met
* Meets standards for Registered ANP working at Advanced Level


Experience

* Experience of Practice within the Four Pillars
* Demonstrable experience of advanced nursing practice - achieving equity with peers working at this level
* Experience of prescribing and undertaking medication reviews
* Experience of working within a Primary Care Setting
