Overview
The Urgent Community Response (UCR) Service provides urgent support, seven‑days a week, to help prevent unnecessary hospital admissions. A two‑hour crisis response is delivered by a multi‑skilled team of professionals who visit patients at home or at their usual residence if their health or mobility is declining and they are at risk of admission.
Teams work closely with the local ambulance service to pull category 3 and 4 patients off the ambulance stack, supporting rapid community response and allowing patients to remain at home. In addition, the UCR service offers a virtual ward supported by digital technology and remote monitoring, delivering hospital‑level care at home safely and comfortably.
Multidisciplinary Team
The team is composed of GPs, nurses, paramedics, pharmacists, occupational therapists, physiotherapists, health and therapy assistants, medics and others who work collaboratively to reduce hospital admissions and facilitate early discharge.
Responsibilities
* Provide clinical expertise and problem‑solving support for the clinical team.
* Deliver clinical care to patients, including intravenous therapy and other interventions.
* Coordinate the multi‑disciplinary team, allocating visits appropriately.
* Develop and improve the service through reflective practice, clinical supervision and audit.
* Identify suitable patients for early discharge from hospital or for preventing admission to the acute sector.
Qualifications & Requirements
* Registered physiotherapist with senior level experience.
* Full UK driving licence and access to a vehicle for the geographic area covered by the team.
* Experience in community or virtual ward settings preferred.
