Job Overview Are you looking for a new challenge? The Thoracic Surgery Nurse Case Manager (NCM) team is looking for a motivated, flexible and hardworking individual to join this progressive team. University Hospital Southampton is a tertiary referral centre of excellence for Thoracic Surgery that works in collaboration with hospitals across the south of England. The Thoracic Surgery NCM role is an integral part of both the medical and wider health care team, involved in all aspects of patient care in the work up to surgery. What You’ll Do Detailed job description and main responsibilities The Nurse case Managers provide support for patients from pre-operative pathway. You will liaise closely with other nurse specialists and MDT teams across the regions to ensure quality care throughout the whole of the patients’ journey. You will undertake holistic nursing and medical assessments of Thoracic Surgical patients, including history-taking and physical examination, in the outpatient setting. You will also perform pre-admission assessment and clerking in preparation for surgery. What We’re Looking For The post holder will ideally have experience of supporting patients with Cardiovascular & Thoracic disease within primary or secondary care. They will need to be adaptable and flexible and work in varied environments across organisational boundaries. You will require a flexible approach to working patterns. You will be expected to have excellent communication skills, promote the delivery of a high standard of patient-centred care and work between organisational boundaries. You will analyse, monitor, and implement actions that ensure the service delivery is safe and effective in line with the Trust values with the support of your colleagues and line manager. Person specification Qualifications / Training Required Essential criteria Registered nurse on relevant part of register Completion of Nursing Degree Evidence of Working towards Masters-level modules in relevant subject or evidence of CPD to M-level equivalence. Specialist training in area of practice. History taking and Physical Assessment Desirable criteria Master’s degree Independent Non-Medical Prescribing PIN Counselling qualification Teaching qualification Leadership training IT training Previous or relevant experience necessary Essential criteria Specialist clinical skills and experience. Evidence of policy development and implementation. Evidence of Change Management, leadership, innovation Evidenced based care philosophy Clinical experience at band 6 grade’/equivalent or above in relevant speciality Commitment to Nurse Led care and further development Desirable criteria Development /use of integrated patient care pathways Experience of nurse –led care/clinics in own practice Multidisciplinary team working. Team leadership role Aptitudes And Skills Required Essential criteria Confident of own knowledge base Proven negotiation skills Ability to synthesise information quickly and act on findings. Excellent communication skills, able to communicate effectively at all levels. Innovative approach to problem solving Able to manage own workload autonomously IT skills (Word processing, email, and internet as a minimum). Desirable criteria Awareness of own leadership skills/style Special requirements of the post Essential criteria Clear about professional accountability re. autonomous practice Able to demonstrate strategic thinking. Flexibility to attend evening meetings/clinics if required, by prior arrangement Desirable criteria Organisational, business and service planning awareness.
