Clinical Responsibilities 1. Develop and maintain a specialist level of knowledge across a range of clinical procedures and practices to use as part of a rotation between clinical placements. This will typically involve specialist knowledge and skills in triage, history taking, structured physical examination skills, problem-solving and clinical decision-making in order to develop a clinical impression/differential/final diagnosis for patients. 2. Work alongside a nominated GP and/or Advanced Clinical Practitioner in providing a first point of contact for patients presenting with undifferentiated, undiagnosed problems across a variety of clinical or non-clinical settings according to patients needs (e.g. at the patients home). Develop clinical skills in assessing complex needs of patients/clients. 3. Evaluate information from clinical examinations and/or triage (incl. remote) in order to initiate appropriate treatment and/or referral. This includes appropriate signposting of patients to plan care for patients. 4. Instigate relevant physiological observations, point of care or diagnostic tests/investigations and interpret these within an agreed scope. Discuss the result and implications of these investigations with patients as necessary. 5. Plan provision of care for patients as appropriate e.g. onward referral to clinical services, self-care advice and prevention advice. Consider holistic aspects and work in accordance with wider guidelines, protocols, policies and patient care pathways. 6. Supply and/or administer medication or treatments in accordance with agreed scope of practice, agreed protocols, any Patient Group Directives (PGD) and Trust formulary. 7. Work in partnership and liaise with multi-disciplinary teams within primary/urgent care, community care and across secondary care, to promote integrated approaches to patient care and seamless patient referral into appropriate healthcare services/pathways. Ensure safe handover of care within and outside GP and NWAS as appropriate. 8. Communicate sensitive or highly sensitive, confidential information concerning patients/clients requiring empathy, persuasion and reassurance (e.g. unwelcome news). Some patients may have special care needs, understanding needs or may be hostile or antagonistic, requiring a specific approach. 9. Display a sound understanding of consent and capacity, ensuring consent is granted for any treatment undertaken and best interests are considered as necessary 10. Where trained and directed to do so, undertake specific duties that are relevant to the practice and/or population needs (e.g. in primary/urgent care). These may include: Participating in the assessment of local community health needs and assisting to develop patient and family-centred strategies. Assisting within the development of new patient services and participation in initiatives to improve or develop existing patient services. Assisting the practice/treatment setting to review emergency care protocols (e.g. by providing updates and training to primary care staff within scope of practice). Participating in outreach, drop-in and surgery based services for patients with long term or routine problems (e.g. immunisation using approved PGDs). 11. Support the education of patients and the public in preventative health issues and relevant health improvement programmes as appropriate 12. Provide role appropriate clinical advice and support to operational resources and EOC staff as appropriate (voice-to-voice and face-to-face). 13. Communicate effectively with other healthcare professionals, colleagues and a range of other partner agencies. This includes liaison with the Police, Fire and other statutory or Government agencies as necessary. 15. Fully document all aspects of patient care and complete all required documentation for legal and administrative purposes in accordance with relevant standards & policies. Work in accordance with administrative systems in practice relating to, but not limited to, the management of clinical data and written information. 16. Report any adverse events or incidents in accordance with agreed local policies and protocols e.g. safeguarding and medicines management related incidents. 17. Empower others within clinical practice and demonstrate an ability to make effective clinical decisions based on recent events and evidenced based practice. 18. Work by appropriate infection control procedures/policies at all times, maintaining work areas in a tidy and safe way and free from hazards. Professional Practice 1. Work at all times within the boundaries of the delegated/agreed duties and scope of clinical practice, at no time undertaking procedures/interventions that have not been agreed by the respective clinical lead/nominated deputy. 2. Contribute to multi-disciplinary, case conferences and practice educational meetings across primary care. Promote a culture of involvement of patients and their carers in all aspects of care provision. 3. Contribute to the achievement of agreed quality, cost and performance targets within placement settings in order to sustain high standards of patient care and service delivery. 4. Contribute to policy development and policy review within practice areas. This will include making comments on proposals for change alongside implementing changes to work practices and procedures in own work areas following review. 5. Represent the Trust/Placement at meetings as delegated by the lead GP or Senior Clinician. Actively participate in primary care practice/unit meetings and training sessions 6. Assist with the development of the service, rotational working models and placements. 7. Participate in research, audit, service development or service improvement projects within all placement settings. 8. Ensure effective time management and prioritisation of your clinical workload across placement settings, escalating any concerns as appropriate. 9. Challenge professional behaviours/practices in a constructive and supportive manner to ensure that patient care & service delivery is of a high standard (e.g. standards of dress or conduct). 10. Use initiative in carrying out any immediately necessary tasks to ensure continued service provision across clinical placement settings in the absence of a more senior clinical or operational lead. 11. Deputise and/or operate under the delegated authority of more senior clinicans or managers as required, expanding on the knowledge, skills and experience for personal professional development.
