Role Summary
We are seeking an experienced Community Nurse (Band 6) to join our Dementia Services team in Telford & Wrekin. The postholder will work closely with individuals who are suspected of or diagnosed with dementia, supporting them and their families within community settings. Key Responsibilities
* Provide high-quality nursing care to patients with dementia in community settings.
* Undertake comprehensive assessments, care planning, and regular reviews.
* Work collaboratively with multidisciplinary teams, families, and carers to ensure coordinated care.
* Promote independence and wellbeing while safeguarding patient dignity.
* Support early diagnosis and timely interventions for dementia patients.
* Maintain accurate records in line with professional and organisational standards.
* Contribute to service development and continuous improvement in dementia care.
Essential Requirements
* Registered Nurse (NMC Registration – Adult/ RMN).
* Previous experience working with individuals with dementia.
* Strong communication, interpersonal, and organisational skills.
* Ability to work autonomously and as part of a multidisciplinary team.
* Commitment to person-centred care and safeguarding principles.
* Previous community nursing experience.
Desirable
* Training in dementia care or related specialisms.
Additional Information
* This is a full-time role for approximately 12 weeks, with an immediate start.
* Candidates must be available to work Monday – Friday, 09:00 – 17:00.
* Placement will be at Severnfields Health Village or Hall Court (Telford & Wrekin, Shropshire).
