Mental Health Nurse Practitioner

Are you interested in leading and participating in crisis care for young people with emotional, wellbeing, mental health or social care needs?
We are seeking passionate, driven nurses to join our amazing collaborative service between Surrey and Borders Partnership NHS Trust and Surrey County Council to deliver mental health assessments for young people requiring respite care and who require wrap around care planning for the next step in their journey. This would involve developing risk formulation, risk management plans and Safety/care planning. There would be expectation that this role would support the young person and the residential team when young people are at Extended Hope House for respite as well as in reach to the acute hospitals in Surrey, Health Based Place of Safety in Guildford and outreach to the community if needed.
Our operational hours are Monday to Sunday including Bank Holidays. Weekend and bank holiday shifts will be expected. Shifts for this post will be 1pm-9pm.
Main duties of the job
This role is on a rota basis and so will include weekends, bank holidays and some unsocial hours during the week.
* working with 11-18 year olds who are in mental health or social crisis, usually the young people have complex needs and work alongside various agencies to meet their care needs
* supporting the families and carers
* mental health assessments
* care planning and review
* risk formulation and management plans
* Safety Planning
* Support to the residential team at Extended Hope House
* Educational support for the residential workers
* Professional meetings which may also include escalation calls, network meetings, care plan reviews and discharge planning review
* Lead and oversee the care and risk of young people who are in the House for respite and advise, support and role model for the team how to manage the situations as they arise
You will work closely with the whole of the Crisis Pathway Network in Surrey in particular the Hope Service

Job responsibilities
To be a core member of the Extended Hope and Extended Hope House Team, based in Redhill. Reporting to the Manager of the provision you will be participating in the provision of urgent assessments, care planning, risk formulations and treatment of children and young people requiring support for emotional, wellbeing, social and mental health needs either through respite at Hope House or through assessment and support by outreaching to community or in-reaching to acute hospitals or Health Based Place of Safety.

* Driving license
Experience
* Extensive experience working with young people in the field of mental health and wellbeing
* Experience of undertaking complex mental health and/or social care-oriented assessments in acute hospital or community settings
* Experience of formulating and implementing care, risk and safety formulations that could be managed within a respite care provision
* Experience of working within a complex health and social care system
