Who We Are
The Reablement service support customers in their own home, whether they have had a stay in hospital or just not managing their daily living tasks at home, with washing/dressing, meal preparation, administration of medication, showering/bathing, mobility, and transfers. We aim to return the customer to their base line of independence.
About the Role
To be a team player who supports, assists and promotes the reablement of customers with identified needs. To therapeutically and proactively enable customers to reach their own potential, maintain their independence and continue to live in their own home.
Key Responsibilities
* Provide support for customers in line with CQC regulations
* Enable, reable and promote independence to customers in line with their goal plan in their home
* Encourage and motivate customers to build confidence, enabling customers to reach their potential, achieve their personal goals
* Support customers with the preparation of meals, personal care, dressing and medication
* Assist and support customers, including those with complex needs and behaviours, with daily living tasks such as personal care
* Maintain clear and accurate records
About You
* A good understanding of rehabilitation
* A proactive, flexible approach to work with a can-do attitude
* An empathetic and caring nature
* NVQ level 2 Health and Social Care would be advantageous
* To be a driver and have access to a vehicle
* The vehicle must be insured on Business use
