Overview
We have an exciting opportunity for an enthusiastic Occupational Therapist to join our established Stroke Early Supported Discharge Service. We work closely with acute stroke services in our area and our neurology colleagues in the community to provide continuity of care across the stroke pathway. Our aim is to promote recovery and well-being, whilst empowering patients to self-manage and maximise their independence using health coaching techniques and meaningful activity. We support safe discharges home and facilitate rehabilitation therapy in patients places of residence. We take a very holistic approach to rehabilitation across the domains of self-care, productivity and leisure. You will have the opportunity to utilise your occupational therapy skills to the full. As well as conducting assessments and devising rehabilitation programmes aligned to patients\ goals, we provide education, emotional support and carer support.
Team and setting
The immediate team includes occupational therapists, physiotherapists, a speech and language therapist and therapy assistant practitioners.
Responsibilities
* Conduct assessments and devise rehabilitation programmes aligned to patients\ goals.
* Educate patients, provide emotional support and support carers.
* Utilise occupational therapy skills to the full within a Stroke Early Supported Discharge service.
* Manage an own caseload while collaborating with the multidisciplinary team and partner organisations.
* Support safe discharges home and facilitate rehabilitation in patients\ places of residence.
Qualifications and experience
* Qualified occupational therapist with post-graduate experience working with stroke patients.
* Ability to manage a caseload and work collaboratively within a multidisciplinary team and partner organisations.

Contract details
Part time, permanent position, 26 hours per week, Band 6. Benefits include a £2,500 Golden Hello payment (taxable), paid in three instalments: £1,000 on appointment, £500 after 6 months in post, and £1,000 after 12 months in post (pro rata if applicable). The Golden Hello is not available to internal applicants.
