About the role: You will join our valued team of Case Managers to support young people and adults following life-changing injuries. As a Case Manager, your focus is the client’s best interests, coordinating care and rehabilitation tailored to their needs. The aim is to enable clients to regain and maintain the best quality of life, working closely with them, their families, and care teams to support their physical, cognitive, and emotional wellbeing. The role is home-based, managing a client caseload with visits within your geographical area. Employment options include part-time salaried or flexible hourly work, from 21 to 30 hours per week. A driver’s license and access to a car are preferred, as travel may be up to 2 hours from your home; travel and mileage are paid.
