Band 6 Clinical Nurse Specialist
The Virtual Ward has been commissioned to provide a safe alternative to manage and monitor a specific group of patients with acute respiratory illness, Frailty or Heart Failure as an alternative to hospital admission and /or to facilitate earlier discharge from hospital. The patients care will be delivered in their own home by utilising cutting edge patient friendly technology to provide remote monitoring, along with additional tailored in person or telephone support.
The role of the virtual ward team is to coordinate the hospital based and community based elements of the ward by
* Promoting the benefits of the virtual ward to patients and HCP
* Identifying and admitting patients to the virtual ward, ensuring seamless transfer from the hospital site to the virtual ward.
* Assessment and remote monitoring of patients in the community
* Delivering Specialist expertise to ensure patients feel supported on the virtual ward
* Employing holistic assessment and strategies to manage emerging health needs.
* Provide administrative duties to facilitate the smooth operation of the service
* To integrate into the wider Respiratory directorate.
Main duties of the job
We have 37.5 hours in the community. The community post holders will be integrated within the home Oxygen service and will be expected to fulfil duties in line with this remit. This will involve case management of Oxygen patients in West Cumbria. Bases are to be agreed, with some home working, but you will be required to regularly attend the clinical base at Wigton Hospital.

Qualifications
Essential
* Level 1 Registered Nursing Qualification or Registered Physiotherapist
* Evidence of continuous professional development to degree level or equivalent experience
Desirable
* Post registration qualification in respiratory or chronic disease management
* Specialist qualification in nursing / AHP equivalent experience in the field
* Post registration qualification in teaching and assessing / NMC mentorship qualification
