What you will do
* Undertake Care Act 2014 assessments within a hospital setting to support safe and timely discharge.
* Facilitate discharge planning in line with Homefirst/Discharge to Assess (D2A) pathways, promoting independence and recovery.
* Complete Mental Capacity Assessments and Best Interest decisions in line with the Mental Capacity Act 2005.
* Work collaboratively with health professionals, discharge coordinators, families, and community services to coordinate care packages.
Person Specification
* Degree in Social Work (BA/MA or equivalent)
* Evidence of continued professional development
* Registered with Social Work England
* Knowledge of relevant legislation and frameworks, including the Care Act 2014, Mental Capacity Act 2005, and hospital discharge guidance.
* Enhanced DBS clearance
