To support the Multi Agency Mental Health Lead who undertakes mental health and risk assessments for frequent callers to the police in the Rochdale borough. To assist in providing follow-up care, making referrals, and working with people in their own homes and other community settings. To assist in liaison, education, support, and advice to police officers/multi-agencies. To participate in data collection and the overall development of the service, ensuring that the principles of Clinical Governance underpin all developments.
Clinical Work
1. Assist the Multi Agency Mental Health Lead in liaising with police to identify frequent callers.
2. Facilitate access to and provide a liaison and link role between Greater Manchester Police staff, frontline professionals, and multidisciplinary teams.
3. Ensure a policy of social inclusion is reflected in interventions provided.
4. Support the Multi Agency Mental Health Lead in ensuring interventions are met from care plans.
5. Proactively outreach to engage service users.
6. Work with individuals in community settings to engage them in treatment and support.
7. Identify the need for protection in line with safeguarding children and the Protection of Vulnerable Adults Policy and comply with statutory obligations.
8. Liaise and negotiate with other professions, statutory, and independent agencies to ensure the best possible service for service users and carers.
9. Collect data to assist in the evaluation of the service.
10. Involve family and carers in care with the service user’s consent.
11. Provide education and appropriate self-help resources tailored to individual needs.
12. Maintain accurate and up-to-date clinical documentation on all service user contacts, ensuring confidentiality at all times.
13. Possess a sound knowledge of issues surrounding violence and aggression, and be able to diffuse potentially difficult and volatile situations.
