Offer description

To work up to 37.5 hours as part of the LPC Clinical Pharmacy team, contributing to the implementation of the Structured Medication Review & Medicines Optimisation DES specification.

The post holder will be working within the Prescription Hub with the member practices of Blackburn with Darwen to deliver efficient repeat prescribing processes thus reducing medicines waste and delivering better quality for patients and the GP practice.

They will support prescribers and general practice staff in improving the health of their patients through the rational and safe use of medicines, and the development and implementation of safe medicines systems.

LPC provide services to CQC regulated environments and as such complies with the VCOD (Vaccination as a Condition Of Deployment for Healthcare Workers) 2021 legislation.


Main duties of the job

Manage Prescription Hub telephone service for patients to use to order medication and make enquiries.

Communicating with patients and all stakeholders regarding prescribed medication, filtering complex queries to senior team members.

Contribute to reviewing, improving and implementing repeat prescribing policies and SOPs.

Promote digital processes to take requests for repeat prescriptions from patients by increasing the number of patients using patient access and Electronic Prescribing service.

Identify over ordering of prescriptions and inform them of any items that they are no longer taking.

Upskilling the team to ensure that all staff are aware of correct procedures.

Review discharge summaries, out of hours notifications, medicines use review and new medicine review forms to ensure accuracy and highlight changes made to clinical team.

To be involved in the Medicines Optimisation workplan to ensure medications prescribed are the most cost effective.


Job responsibilities

Reducing Medicine Wastage

Highlight over and under ordering of repeat medicines to the appropriate clinician or staff member as per SOPs.

Synchronise medication quantities to reduce footfall in the practice.

Remove from patients repeat list any unnecessary repeat prescriptions that have not been ordered for an agreed length of time, as per SOP.

Check each individual item is needed when a patient asks for all their repeat medication.

Check that medicines that patients take when required are needed e.g. painkillers, inhalers, creams are moved to the variable repeat part of the patients medical record, as per the practice SOPs.

To promote patient education and awareness regarding the issues of prescribing waste, self‑care.

To work to all SOPs in relation to the repeat prescribing systems and refer to them as necessary.

Highlight areas of prescribing waste and suggest methods for reduction.

Identify areas of care home waste and work with the care homes to reduce unnecessary prescribing.

Improve patient and staff satisfaction

Implement ways to promote patients to take responsibility for their repeat prescription ordering and to highlight any problems associated with taking their medicines.

Offer medication reviews with PCN clinical pharmacists or Medicines Use reviews with the community pharmacist, if a patient is experiencing problems with their medicines.

Provide guidance to practice staff, and promote communication throughout the practice team.

To implement a process within the Hub for signposting patients to relevant services as part of a care‑navigation programme for reducing prescriptions for over the counter medication.

Ensure that when queries are generated that they are highlighted through the appropriate channels, to ensure they are resolved in a timely manner and before the next repeat prescription is due.

Ensure clear dosing instructions appear on all repeat prescriptions.


Person Specification


Experience

* Experience working with GP clinical systems e.g. Emis Web; Able to work within SOPs with high levels of concentration; Able to manage own workload and work independently; Is sensitive to patients circumstances, such as chronic disease and terminal illness and distressed patients and aggressive patients.
* Experience working in both primary, secondary care and community pharmacy.


Additional

* Good attendance record; Ability to be flexible and adjust to changing service requirements.
* Willingness to travel within locality as required.


Personal Skills and Attributes

* Good written and verbal communication skills; Basic computer skills; Excellent interpersonal skills; Able to work in a team or on own initiative, and to plan own time effectively. Positive and professional image.
* Familiarity with Microsoft Office packages.


Qualifications

* Good standard of general education. BEC (NVQ level 3 Pharmaceutical Sciences), City & Guilds Pharmaceutical Science, or equivalent. Registration with the General Pharmaceutical Council.
* Evidence of ongoing education/training. ECDL.


Knowledge

* Good working knowledge of drug therapies; Understanding of NHS.
* Familiarity with GP computer systems; Familiarity with ePACT.net software.
