As Cambridgeshire Community Services prepares to merge with Norfolk Community Health and Care Trust to become one of the largest community providers in the country, an exciting opportunity has arisen to help lead this programme of work as the Assistant Director of Integration and Transitions.

The role is a key leadership position in the Strategy and Transformation Directorate, responsible for coordinating all aspects of the work programme to fully integrate the two organisations through the post‑transaction integration plan (PTIP). It also involves leading and managing transition programmes for the merger and the creation of the new Trust, and managing transitions in and out of the organisation within agreed timescales and budgets.


Responsibilities

1. Manage and facilitate transition to new pathways and processes across the organisation.
2. Manage programme and project delivery in collaborative and matrix working methodologies.
3. Implement and manage appropriate governance for the programme, including Programme Board, project and workstream groups.
4. Develop and manage the transition programme for the merger into the new organisation from 1 April 2026.
5. Lead projects and programmes as required or as delegated by the Director of Strategy and Transformation.
6. Be the day‑to‑day lead on behalf of the Executive Lead for the successful delivery of the programmes, outcomes and benefits.
7. Plan and design the transition and other programmes and projects. Proactively monitor their overall progress, resolve issues and initiate corrective action as appropriate.
8. Define and deliver the programme governance framework, including the development of work plans and risk and issues registers and logs.
9. Effectively coordinate the projects and their interdependencies, with the programmes and with other projects.
10. Maintain the overall integrity and coherence of the programmes. Develop and maintain the work stream environment to support each project and programme.
11. Manage the programme budget, monitoring expenditure and costs against the benefits and ensuring these are realised as the programme develops.
12. Facilitate the appointment of individuals to the project delivery teams.
13. Ensure the delivery of each programme meets the workstream requirements and is of appropriate quality, on time and within budget.
14. Ensure maximum efficiency in the allocation of resources and skills within the project’s portfolio.
15. Manage third‑party contributions to the programme.
16. Initiate extra activities and other management interventions wherever gaps in the programme are identified and issues arise.
17. Report progress through committees and groups defined in the governance framework.


Communication

1. Represent the organisation at appropriate local, national and regional forums, developing a network of contacts and knowledge to support the design of services, strategies and to promote the organisation in the local economies. This will include relationships with Integrated Care Boards, local authorities, service commissioners, other NHS bodies and deputising for the Director when required.
2. Communicate complex information to staff, including information that will not be received positively, e.g. service closures, resulting in job losses or changes. This will require effective interpersonal skills and the development of relationships with a variety of stakeholders.
3. Provide reports and papers to Executive and Board committees, presenting them as required.


Budgetary Responsibility

1. Authorised signatory for service and programme budgets.
2. Budget manager for programmes and team.


People Management

1. Responsible for day‑to‑day management of the transition team and leadership of transition project teams.
2. Line manager for the Transition Team, ensuring appraisal and development and compliance with training and other requirements.
3. Lead and support the development of programme management principles within the team, across the wider leadership team and across the Trust.
4. Maintain own professional development and requirement to take part in appraisal and KSF process.


Clinical and Practice Governance

1. Observe and maintain strict confidentiality with regards to any patient, family, staff, records and information in line with the Data Protection Act.
2. Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.
3. The post holder must adhere to the Trust risk assessment and risk management processes.
4. The post holder must adhere to infection control policies and procedures.
5. Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.
6. The post holder must participate in clinical and safeguarding audits as required.
7. The post holder is required to participate in relevant emergency preparedness process for their team.
