Main duties of the job
The post is community based with most work being undertaken at Cromwell House. This will include new patients, follow up appointments, attending the weekly meeting with the CMHT, taking part in CPA meetings and conducting home visits. The post holder would be expected to see emergency assessments following a duty assessment (by care co-ordinator rota) and would also be expected to engage in Section 12 work for patients allocated to their teams. The CMHT has the acute assessment role of a crisis team as per the local Acute Care Pathway (ACP) which means that all referrals to secondary services are triaged in community teams and assessed before referral on. The post-holder would provide advice and support to other members of the team, primarily for their own case-load but also for new patient queries and general advice.
