Overview

A high profile, accessible and responsive leader with managerial responsibility for the delivery of the Critical Care Outreach Service within the Trust. Service provision will facilitate improvements in the quality and provision of services through collaboration between Multidisciplinary Teams and other service agencies involved in caring for acutely ill patients. The post holder will lead service developments and act as a resource for other healthcare professionals, improving the quality of care provision whilst carrying a specialist caseload of clients and providing advice to patients, carers and the wider multidisciplinary team.


Responsibilities

* Accountable for own actions in accordance with Code of Professional Conduct.
* Provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
* Operationally lead the Critical Care Outreach Team and provide advanced clinical expertise, knowledge and skills using clinical judgement and decision making in the care and management of acutely unwell patients.
* Demonstrate highly developed communication skills enabling maintenance of a high quality service and ensuring timely management and resolution of unexpected events and conflicts in priorities.
* Coordinate the care of critically ill patients through the critical care pathway to ensure effective management of their patient group.
* Identify and assess complex clinical and non-clinical issues, implement resolutions and monitor/audit outcomes.
* Undertake advanced health assessment using history taking and advanced physical assessment knowledge to ensure optimum patient management, including patient monitoring, equipment management and defined expanded clinical roles.
* Develop relevant treatment plans for patients requiring intervention including fluid resuscitation, oxygen therapy in accordance with patient group directives, and ordering of appropriate laboratory and radiology investigations.
* Facilitate discharge from critical care to support continuing recovery, coordinating all aspects of care with the multi-disciplinary team; manage and coordinate patients discharged from critical care to wards with a tracheostomy (including weaning and decannulation).
* Ensure provision of a high quality professional nursing service across the Critical Care Outreach service, promoting best practice and implementing a review process to meet patient and service needs.
* Support the Critical Care Services in the development and implementation of policies, procedures and guidelines across the Trust service, ensuring clinical governance is embedded in practice.
* Maintain budget responsibility for the Critical Care Outreach Team for nursing monitoring variance and implement contingency plans as appropriate, ensuring service provision within the agreed financial resource.
* Support the Clinical Lead and General Manager in ensuring service objectives are met; support the Modern Matron in developing nursing services to meet standards, targets and objectives.
* Manage processes efficiently (e.g., critical incidents, complaints, staff registration, training and development; and appraisal).
* Utilise information and IT systems to produce accurate and timely workforce and resource data and reports.
* Develop, implement and evaluate projects to improve patient care and service development through the audit cycle, linking to the clinical governance framework and business planning process.
* Undertake direct workload requiring moderate to high physical effort according to patient dependency or staff need.
* Demonstrate leadership to provide professional and emotional support to individuals and groups during difficult situations or unexpected events.
* Adhere to Health and Safety and Trust policy and Divisional objectives to protect healthcare staff and service users from clinical and non-clinical risk.
* Contribute to and participate in the Trust’s education strategy.
* Any other duties/tasks reasonably expected by the Trust.
* Represent Nursing at Trust, Regional and National level.
