We are seeking a dedicated and experienced Qualified Social Worker (QSW) to join our Hospital Pathway Team in Sutton. This is an exciting opportunity to make a significant impact on the lives of individuals transitioning from hospital to home care, ensuring they receive the support they need during this critical time. The position offers a competitive rate of £32 per hour on an umbrella basis, and the successful candidate will play a pivotal role in facilitating safe and effective discharges.
Key responsibilities include:
* Conducting comprehensive assessments of patients’ social care needs and risks.
* Developing and implementing care plans in collaboration with healthcare professionals, patients, and their families.
* Coordinating with various agencies to ensure a seamless transition from hospital to community services.
* Providing emotional and practical support to patients and their families during the discharge process.
* Maintaining accurate and up-to-date case records in accordance with legal and organizational standards.
Essential qualifications and skills required for this role include:
* A recognized social work qualification (e.g., CQSW, DipSW, or equivalent).
* Current registration with Social Work England.
* Proven experience in a hospital or community social work setting.
* Strong assessment and intervention skills, with a focus on safeguarding.
* Excellent communication and interpersonal skills, with the ability to work collaboratively in a multidisciplinary team.
