We are looking for enthusiastic and adaptable General Practitioners to join the Hereford GP Hub, an evolving and innovative service delivering accessible, high-quality care across Herefordshire.
The Hereford GP Hub offers a blend of face-to-face and virtual appointments, giving GPs the opportunity to work in a flexible, modern care model that supports patients with timely access to treatment and advice. Working as part of a multidisciplinary team alongside Advanced Nurse Practitioners (ANPs),you will playa central role in delivering responsive, patient-centred care.
This role offers a varied and rewarding caseload, with a balanced mix of face-to-face and virtual consultations across the week. As part of a revised rota model. You will also contribute to a service that works closely with wider healthcare partners, supporting smooth patient pathways and joined-up care.
The service operates from 9am to 6pm, with flexible morning, afternoon and full-day sessions available. Sessions include a mix of15-minute bookable appointments for NHS 111,Emergency Department and GP surgeries with clinical consultations for allocation admin time.

We are recruiting for up to 24 GP hours per week, with part-time (permanent and fixed-term) and casual (0 hours) roles available within the following service hours:
Mondays 14:0018:00 (virtual appointments)
Tuesday 09:0013:00 & 14:0018:00 (virtual appointments)
Fridays 09:0013:00 (face to face) & 14:0018:00 (face to face & virtual)
Location: Nelson House (face to face) & home working (virtual)

Job responsibilities
The post holder will be responsible for seeing patients in an identical manner to regular general practice work, taking responsibility for a patients care and only referring back to their own GP where appropriate, consistent with ensuring every contact counts.
This will involve routine, urgent and telephone appointments, as well as supporting the clinical and administrative teams in the best interests of the patient that supports a patient seeing the right person at the right time in the right place.
The GP is expected to do any referrals required and appropriately prescribe within the Herefordshire guidelines, taking into account the Low Priority Treatment guidance from the CCG. To assist in development of the service, with incident reporting and suggestions for improvements through the appropriate system.
