Band 4 Haematology Macmillan Care Support Worker

Overview
The Haematology department is looking for an enthusiastic, forward‑thinking care support worker to join the team.
Responsibilities
The care support worker will work with the Haematology CNS team to provide care to Haematology patients and coordinate care for people with non‑complex care needs. This includes:
* Rapid re‑entry into the system for those identified with urgent or specialist needs, providing general information and support about cancer services and helping patients navigate the health and social care system.
* Triage incoming calls and initiate appropriate responses according to protocols and individual pathways, using good communication skills, basic clinical awareness, and appropriate tools and procedures; liaising as appropriate when non‑routine and referring complex decisions to the team for assessment and review.
* Coordinate the care for a defined group of patients assessed by a registered practitioner as having level‑one care needs for supported self‑management.
Administrative duties
The role involves working in an outpatient setting with administrative responsibilities such as booking appointments, taking telephone calls and supporting the Haematology clinical nurse specialist in the clinics.
Accountability
The Haematology Macmillan Care Support Worker will be responsible and accountable for their practice and behaviour under the guidance and supervision of a registered practitioner and be a member of an existing team.
