This role supports people to live ordinary lives in their own homes, focusing on independent living and personalised support.

Job Description / Person Specification
The role works within the Urgent & Emergency Care (UEC) Teams and follows a Discharge to Assess model with partners from Acute and Community Health. Referrals originate from the three acute hospital trusts within Nottinghamshire. The team collaborates with transfer of care hubs at Kings Mill Hospital, Queens Medical Centre and Bassetlaw Hospital to plan supported discharges and short-term social care support as needed. After discharge, the aim is to continue recovery, rehabilitation and reablement with assessments to promote independence and well-being. You will:
* Identify a person’s goals and coordinates a range of outcomes, including reablement and enablement opportunities and access to services.
* Use strength-based approaches to promote choice, personalisation and self-directed support.
* Collaborate with Social Workers, Occupational Therapists and Community Care Officers in a fast-paced, supportive team.
* Manage a mixed-priority workload with good organisational skills.

Essential: recognised social work qualification, registration with Social Work England, knowledge of relevant social care legislation; experience in adult safeguarding and partnership working with health and external agencies. You may be required to work flexibly across UEC teams to manage pressures and achieve outcomes. Weekend and bank holiday cover in the transfer of care hub is on a voluntary rota basis for enhanced pay.
