The Colte Partnership have an exciting opportunity for a care co-ordinator to join our friendly team, working at our Ardleigh Surgery in Essex. The role is offered on a full-time basis and involves working very closely with the practice team and is pivotal in ensuring all patients receive the best possible care and service. The successful applicant will: Ensure a high standard of service delivery. Be a key point of contact for both patients and carers, supporting their journey through primary care. Support the practice in coordinating all key activity including access to services and advice. Book appointments and run searches to identify vulnerable patient groups. Assist in running the local Dementia café. Manage the populations health initiatives and campaigns. About the Candidate To be considered for this position, applicants should have the following: Excellent interpersonal communication skills. Be able to speak professionally and confidently to different patient groups, colleagues and clinicians. Understand primary care or have been in an NHS setting in the past. Be proactive and able to work independently, using their own initiative, but able to take and respond to detailed instruction where required. Have excellent record keeping and IT skills. Be able to prioritise multiple tasks and achieve them all to a high standard. Previous experience in a healthcare setting is desirable but not essential.
