Job Summary
The role of Mental Health Support Worker / Practitioner within Primary Care is to provide support to people with diverse mental health needs that are being assessed and cared for by Primary Care. This may include referring to onward mental health support from NHS, signposting to third sector support and potentially developing outreach/groups for cohorts of local service users. The role navigates the mental health system between primary care and other services to ensure user is fully supported and can access additional help. All interventions are administered under indirect supervision by Northamptonshire Talking Therapies registered and accredited practitioners and through the community of practice of PCN‑based mental health workers that will be established once these MHSPs are in post. The Mental Health Support Practitioners will contribute to ongoing assessment and treatment planning for service users they are working with.
Main Duties
* Avail county wide across Northamptonshire; expected to spend one day at Lotus House in Northampton and the remaining four days at allocated GP surgeries county‑wide.
* Develop therapeutic relationships with service users and carers within primary care and support access into secondary care.
* Liaise and share information with others involved in care in line with the Consent to Share Information Policy.
* Engage service users with agreed treatment plans and ensure access to appropriate services on a needs‑led basis.

Person Specification
Essential
* Experience of working in mental health.
* Have worked in a fast‑paced mental health service.
* NVQ level 3 or equivalent knowledge and experience.
Desirable
* Experience in completing mental health assessments.
* Psychology degree.
