JOB-20240819-db742659 Adults Social Worker - Older People & Physical Disabilities Team Job Title: Adults Social Worker - Adults Specialism Specialism: Adult Social Care - Older People & Physical Disabilities Location: Reading Salary: £38.00 per hour Contract Type: Ongoing Locum Working Pattern: Part-Time / Full-Time Role Overview An exciting opportunity has arisen for an experienced Adults Social Worker to join the Older People & Physical Disabilities (OP & PD) Team within Adult Social Care at Reading Borough Council. In this role, you will support adults with age-related needs, long-term conditions, and physical disabilities, helping them maintain independence and improve their wellbeing. You will undertake strength-based assessments, develop person-centred care plans, and work closely with multidisciplinary partners to ensure individuals receive the right support and services. Varied Caseload Work with a diverse range of service users and cases, ensuring engaging and meaningful daily work. Key Responsibilities / What You Will Do Conduct comprehensive needs assessments for older people and adults with physical disabilities in line with the Care Act 2014. Develop and implement person-centred care and support plans that promote independence and improve quality of life. Manage a diverse and complex caseload, ensuring timely completion of assessments, reviews, and required interventions. Undertake safeguarding enquiries and risk assessments to protect vulnerable adults from harm. Work collaboratively with health professionals, occupational therapists, housing teams, and community services to deliver holistic care. Support individuals and families in accessing community resources, support services, and managing direct payments. Contribute to hospital discharge planning to ensure safe and effective transitions back into the community. Maintain accurate and up-to-date case records in line with council policies and statutory responsibilities. Requirements Essential Degree or Diploma in Social Work. Registration with Social Work England. Experience working within Adult Social Care services. Strong knowledge of the Care Act 2014, safeguarding procedures, and strength-based practice. Ability to manage complex caseloads and work effectively within multidisciplinary teams. Desirable Experience working within Older People or Physical Disabilities teams.
