Paramedic / Paramedic Practitioner in Primary Care

While advertised as a full-time permanent role, we also welcome applications from paramedics seeking bank, part-time, or fixed-term opportunities

We are seeking a motivated Paramedic Practitioner (Specialist/Advanced Paramedic) to join our growing team. We also welcome applications from Paramedics with a strong interest in Primary Care who are keen to develop their careers, with opportunities for further study, apprenticeship support, and progression into practitioner roles.

You will act as a first point of contact for patients requiring same-day, undifferentiated urgent care. Duties may include home visiting, triage support, or clinic-based work, offering variety and scope to enhance your skills. Responsibilities include initial assessments, forming differential diagnoses, and, with support from the Duty Paramedic Practitioner and GPs when needed, implementing safe and effective management plans.

You will work closely with community nurses, GPs, and members of our expanding Integrated Care Teams to deliver high-quality, patient-centred care.

Main duties of the job

We are dedicated to enhancing patient care and improving the health of our local population. The NHS has changed following the pandemic, but our focus remains on supporting patients in the community and helping to reduce unplanned emergency hospital admissions.

As a Paramedic Practitioner, you will be a first point of contact for patients with same-day urgent care needs. Duties may include home visiting, triage support, and clinic-based care. You will carry out assessments, develop differential diagnoses, and, with support from colleagues when required, implement safe and effective management plans. Working closely with GPs, community nurses, and members of our Integrated Care Teams, you will contribute to high-quality, patient-centred care.

Job responsibilities

The postholder is an experienced Specialist/Advanced Paramedic, who acting withintheir competence, will provide care for the presenting patient from initialhistory taking, clinical assessment, diagnosis, treatment and evaluation oftheir care. They will demonstrate safe clinical decision-making and expertcare for patients within primary care. There is a requirement to seeemergency patients and this may be either in a practice or in the patientshome. The practitioner will need to prioritise and triage the needs ofpatients accordingly making any necessary referrals for investigations in theappropriate manner.

Job responsibilities:

Diagnosingand treating patients presenting with undifferentiated primary care demand:

Triage and treat patients wishing to see ahealth care professional making any necessary referrals to other members ofthe primary health care team

Ensure clinical practice is safe and effectiveand remains within boundaries of competence and acknowledges limitations

Make professionally autonomous decisions forwhich he/she is accountable

Provide first point of contact within theservice for patients presenting with undiagnosed problems, making use ofskills in history taking, physical examination, problem-solving and clinicaldecision-making to establish a diagnosis and management plan

Provide health education / health promotion topatients, relatives and carers.

Refer patients to other members of the primaryand secondary health care team as necessary

ECG (Lead II interpretation)

Minor injuries including wound care

AdmissionAvoidance Work

Accept referrals from other Health CareProfessionals, providing telephone/face to face support and assisting withclinical management.

Follow up phone calls and consultations withpatients, relatives and other Health Care Professionals regarding a patientscare.

Creation of patient specific care plans toreduce avoidable hospital admissions, especially out of hours.

Intercept 999 calls by utilising the SECAmbIBIS system, either performing telephone triage call / visit to reduceavoidable hospital admissions.

Pathologicalspecimens and investigatory procedures

Undertake the collection of pathologicalspecimens.

Perform investigatory procedures eitherclinically indicated or requested by other members of the primary health careteam.

Administrationand professional responsibilities

Maintain their professional registration inline with HCPC standards.

Maintain a high personal standard ofprofessionalism.

Participate in the administrative andprofessional responsibilities of the Paramedic Practitioner team.

Ensure accurate and legible notes of allconsultations, treatments and referrals are recorded in the patients electronicnotes.

Ensure the clinical computer system is kept upto date with accurate details recorded and amended when required.

Ensure expense claims are made accurately,reporting any problems to the Office Manager.

Ensure collection and maintenance ofstatistical information required for regular and ad hoc reports and audit.

Attend and participate in meetings asrequired.

Restocking and maintenance of clinical areasand administrative rooms used.

Supplies andequipment

Maintenance of equipment and stock relating topatient care.

Training andpersonal development

Training requirements will be monitored byyearly appraisal and will be in accordance with team/locality requirements.Personal development will be encouraged and supported by the organisation. Itis the individuals responsibility to remain up to date with recentdevelopments.

Participate in the education and training ofstudents of all disciplines and the introduction of all members of staffwhere appropriate.

Maintain continued education by attendance atcourses and study days as deemed useful or necessary for professionaldevelopment ensuring HCPC requirements are met.

If it is necessary to expand the role toinclude additional responsibilities, full training will be given.

Develop and maintain a Personal Learning Plan.

Completion of annual mandatory online andbasic life support training.

Liaison andCommunication

Work with multi-disciplinary practice teamswithin the locality Practices

Work with the wider multi-disciplinarycommunity integrated teams working with the Locality

As well as the nursing teams there is a needto work closely with reception and office staff to ensure the smooth runningof the practices, reporting any problems encountered to the relevantperson.

There is also the need to establish andmaintain good liaison with other surgeries and agencies including secondarycare.

The post-holder will establish and maintaineffective communication pathways with colleagues, patients and their carers,recognising the need for flexibility to overcome different levels ofunderstanding and cultural backgrounds.

Confidentiality:

Maintain confidentiality in accord with HCPCand local organisation and practice confidentiality policies.

Health &safety:

The post-holder will implement and lead on afull range of promotion and management their own and others health andsafety and infection control as defined in the organisation Health &Safety policy.

Correct usage of Personal Protective Equipment(PPE).

Identifying the risks involved in workactivities and undertaking such activities in a way that manages those risksacross clinical and patient process

Safe management of clinical waste and sharpsprocedures including use, storage and disposal

Using appropriate infection controlprocedures, maintaining work areas in a tidy, clean and sterile, and safeway, free from hazards. Initiation of remedial / corrective action whereneeded or escalation to responsible management

Actively identifying, reporting, andcorrection of health and safety hazards and infection hazards immediatelywhen recognised

Keeping own work areas and general / patientareas generally clean, sterile, identifying issues and hazards / risks inrelation to other work areas within the business.

Spillage control procedures andmanagement

Decontamination control procedures, managementand equipment maintenance

Generate EPS and FP10 prescriptions withinscope of practice, considering local and national guidelines including butnot limited to:

oSouth Central Antimicrobial Network Guidelines

oNICE guidelines

oRoyal Pharmacological Society PrescribingCompetency Framework

Prescriptions outside of scope of practicewill be valid in the following circumstances:

oLife threatening emergencies

oClinical Management Plan (prescribing as asupplementary prescriber)

oEXCEPTany medicines governed under the Misuse of Drugs Act as these cannot beprescribed by Paramedics until the law is amended.

Maintain accurate EMIS records ofprescriptions issued / generated.

Maintain up to date with prescribing byundertaking continued professional development including but not limited to:

oAttending Prescribing Forums

oDiscussing cases routinely with GeneralPractitioners / Non-Medical Prescribers

oReviewing updated guidance

Be part of the regular review of the prescribing scope of practice.

Provide support and guidance to otherParamedic Prescribers.

When required, act in role of Duty ParamedicPractitioner for the Rapid Home Visiting Service.


Person Specification


Qualifications

* HCPC Registration as Paramedic with no restrictions on practice
* MSc Advanced Clinical Practice qualification or equivalent recognised specialist/advanced practice qualification at minimum degree level
* Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and ongoing competence in all aspects of role
* Evidence of working autonomously and as part of a team
* Proven ability to evaluate the safety and effectiveness of their own clinical practice
* Placement / employment experience within primary care
* Post-registration study relating to urgent care / minor illness / minor injury
