Job Summary

We are looking to recruit an experienced care coordinator/key worker to work as a member of the multidisciplinary Community Locality Team. The role will require the post holder to:

To provide effective care and specialist advice for people with psychosis and complex mental health needs and their carers, in order to support their well-being and recovery. This includes depot administration and regular monitoring of mental state.

To liaise with other agencies in planning and assessing programmes of care

Provide formal, on-going clinical supervision and caseload management supervision to junior and less experienced community mental health staff, as directed.

To undertake duties designed to extend clinical and managerial expertise as delegated by the manager when required.

To complete full mental health assessments and complete Dialog+ as part of KPIs.

To produce holistic risk assessments

To regularly liaise with MDT and patients, including home visits, to ensure safety and wellbeing.

Main duties of the job

We treat with compassion, dignity and respect, embedding our trust values in everything we do. On a day to day basis, we take part in regular MDT meetings and morning safety huddles. We also promote clear communication with GPs, families and carers as well as local authority. We work together as a team, building a safe environment, where colleagues are respected and encouraged to take on an active role in MDT meetings.

Qualifications/ Registrations

Registered Mental Health Nurse,

Desirable

Relevant qualification in Psychosocial Interventions with people with severe and enduring mental health problems.

Skills/ Abilities

Significant years post registration experience, at least one of which will be working with people with severe and complex mental health problems.
Demonstrable experience and knowledge of exercising clinical responsibility for patients' care and treatment, both as a professionally qualified practitioner and also within the context of a multidisciplinary care plan and under supervision

Desirable

Experience of teaching, training and/or mentoring.
