Community Mental Health Practitioner (Nurse/OT/SW)
Midlands Partnership University NHS Foundation Trust is recruiting a Band 6 Mental Health Practitioner for the Older Adult Community Mental Health Team in Telford, Shropshire. The role supports multidisciplinary care for older adults with functional and/or organic mental health needs.
Job Overview
Provide specialist assessment and care planning, evaluate treatment plans, triage referrals, and coordinate team activities to promote recovery, independence and quality of life. Location: Telford, Shropshire.
Key Responsibilities
* Provide specialist assessment and care planning in conjunction with service users and carers.
* Evaluate and adjust treatment plans.
* Triage and assess referrals in liaison with multidisciplinary colleagues.
* Coordinate the work of the team and manage complex caseloads.
* Administer medication and comply with Trust procedures.
* Deliver pharmacological and psychologically informed interventions.
* Complete robust risk assessments and support positive risk taking.
Qualifications and Training
* Recorded/registered qualification in CMHN, Social Work, Occupational Therapy, or Psychological Therapy.
* Post‑qualification experience with complex mental health needs.
* Qualification in psychological interventions (e.g., DBT, SCM, CBT, EMDR, MBT) desirable.
Experience
* Experience delivering therapeutic interventions to patients with dementia and their carers.
* Experience assessing, planning, coordinating, and managing people with mental health problems in a community setting.
* Experience working with older adults’ complex and challenging needs.
* Minimum one year post‑registration experience in a mental health setting (desirable).
* Experience supervising junior staff or students (desirable).
