Liaison Psychiatry Senior Clinical Practitioner

The purpose of the job is to provide a hospital-based mental health service to patients of the Acute Trust. The post holder will assess referrals made from the district hospital, work with a small caseload of complex clients, and liaise with referrers involved. They will offer training, consultation, and advice to colleagues working in the Acute hospital setting. The post-holder will enable timely triage, assessment, and intervention for those with co-morbid physical and mental illnesses, ensuring appropriate treatments are accessed where available.

Assessments will be completed within the framework of 1 hour for triage and 4 hours for a full LPS assessment.

As an expert practitioner, the post holder will also provide clinical supervision for band 6 practitioners.


Main duties of the job

Clinical: To provide biopsychosocial assessments of patients attending the hospital with known or suspected mental health problems, including dementia or delirium. Share expertise on mental health crises in over 65s. Work closely with the Acute Hospital Trust and Kent and Medway Partnership Trust teams to promote effective clinical care and good working relationships. Assess social needs that may be greater than mental or physical health needs and coordinate appropriate care plans. Demonstrate risk management and safeguarding practices, practicing safely and according to individual patient needs. Formulate comprehensive, multidisciplinary, integrated care plans and communicate these effectively. Involve carers when appropriate.

Assess and anticipate individual needs in crisis, taking appropriate action and communicating with relevant agencies. Report incidents as per Trust guidelines.

The post holder will work autonomously in assessments, care planning, and data collection.

Job responsibilities

Assess patients with suspected or known mental health issues, including dementia or delirium. Share expertise on mental health crises in over 65s. Collaborate with hospital and community teams to promote effective clinical care. Assess social needs and coordinate care plans. Practice safely with risk management and safeguarding, involving carers as appropriate. Work independently in assessments and care planning. Develop links with hospital colleagues, provide specialist assessments and interventions for adults with co-morbid illnesses, and act as an educational resource. Maintain high standards of practice and evaluate them regularly. Support service improvements, clinical audits, and incident reporting. Lead in specific clinical areas such as Neurodiversity, Triage, substance misuse, dual diagnosis, Dementia/Delirium, and others. Lead on clinical development of band 6 staff and build trusted relationships with hospital colleagues. Complete clinical inductions and promote understanding across services. Engage in service improvements with increased service user and carer involvement, including PREM & CREM feedback.

Communications and working relationships: Communicate effectively with the liaison team and hospital staff. Maintain accurate records, provide assessment reports, and liaise with other professionals and agencies.


Person Specification


Qualifications and Registration

* Registered Mental Health Nurse with NMC pin
* Post-registration therapy qualification or willingness to pursue one
* Masters level education or equivalent experience
* Additional specialist qualifications in areas like ASD, Dual diagnosis, Dementia, etc.


Experience

* At least one year of senior nurse experience (band 6 or above)
* Experience working with individuals using therapeutic approaches like CBT or in community mental health
* Experience managing clinical risk

This role requires a DBS check in accordance with the Rehabilitation of Offenders Act 1975.
