Job summary
We are seeking a passionate and dedicated Occupational Therapist to join the North Home First rehabilitation service. You will be responsible for delivering high quality rehabilitation to patients with a range of physical health conditions including MSK, Stroke, Parkinson's disease, Neurological conditions, Spinal Injuries, Cancer, Post Surgery, General Frailty, Amputees, plus a wide range of other conditions which provides you with a varied and exciting experience within our service.
The post holder will be part of the Allied Health Professions (AHP) service providing Occupational Therapy and will work in a variety of settings.

Main duties of the job
The role involves managing a caseload of adults in close liaison with the Home First Team. Many of these patients will have multiple complex health needs.
1. Be competent as appropriate to the requirements of the role

3. Be a specialist Occupational Therapist and have a detailed professional and clinical
4. They will be a specialist practitioner and have a focus in clinical They will be a specialist practitioner and have a focus in one of the following areas; rehabilitation, integrated community, community intervention and reablement.
5. Be responsible for a defined population of service users in a variety of settings across the Trust including the community and hospitals
6. Be skilled in the assessment of complex/specialist care/support needs and in the development, implementation and evaluation of care/support packages.
7. Have supervisory, training and leadership responsibilities where appropriate and will have some responsibility for team performance and service developments/improvements.
8. Demonstrate initiative and be creative in finding solutions to problems
Work with the team leader undertaking delegated tasks in team management.

23. Registration with HCP/NMC
