Qualified Adult Social Worker needed for our Hospital & Discharge to Assess Team, paying £32.33 ph!

About The Role:
Bournemouth, Christchurch and Poole Council is seeking an experienced and motivated Qualified Social Worker (Adults) to join our busy Hospital Social Work Team based at University Hospitals Dorset, working across:
* Royal Bournemouth Hospital
* Poole Hospital
This is a varied and dynamic role within a fast-paced environment. The successful candidate will work within the acute hospital setting and support individuals transitioning back into the community, including undertaking Discharge to Assess (D2A) follow-up work.
You will need to be confident operating across both hospital and community environments, adapting to changing service demands while ensuring safe and timely discharges.
Key Responsibilities:
* Undertake strengths-based assessments under the Care Act 2014
* Complete capacity assessments and Best Interests decisions in line with the Mental Capacity Act
* Facilitate safe and timely hospital discharges
* Work collaboratively with NHS colleagues and multi-disciplinary teams
* Support Discharge to Assess pathways, including community follow-up visits
* Develop person-centered care and support plans
* Manage a complex and varied caseload
* Undertake safeguarding enquiries where required
Requirements:
* Qualified Social Worker (Social Work England registered)
* Significant post-qualified experience within Adults Social Care
* Recent hospital discharge and/or community experience
* Strong knowledge of Care Act 2014, Mental Capacity Act, and safeguarding legislation
* Ability to work independently within a fast-paced hospital environment
* Excellent assessment, risk management and communication skills
