Honiton Surgery is looking for an experienced Pharmacist to join our practice team. Based at Honiton Surgery, you will work with our GPs and other clinicians to support prescribing and review medication. While working directly for the surgery, you will also collaborate with and be supported by our PCN team of pharmacists and pharmacy technicians.
We are advertising for 1 WTE, but applications from candidates interested in a part-time role of no less than 0.6 WTE are welcome.

Main duties of the job
As a Clinical Pharmacist, you will:
* See patients with single or multiple medical problems requiring medicine optimisation, e.g., diabetes, hypertension, heart failure.
* Carry out structured medication reviews for patients identified as a priority by the practice or through the PCN DES.
* Conduct medicines reconciliations across care boundaries to ensure timely and appropriate medication use with correct monitoring.
* Answer prescription and medication queries from patients and staff across the health network.
* Lead and present improvement projects on PCN-wide policies and procedures.
* Contribute to practice and PCN meetings, GP educational sessions, and other relevant meetings.
* Support GP prescribing leads in implementing changes, maintaining standards, and achieving prescribing targets, e.g., through the Devon Medicines Optimisation Scheme.

Job responsibilities
* See patients with complex medication needs, review ongoing medication plans, and recommend improvements.
* Perform structured medication reviews, including home visits and liaising with secondary care.
* Conduct medicines reconciliations and respond to medication queries.
* Lead and contribute to quality improvement projects and meetings.
* Support prescribing standards and targets, liaise with community pharmacies, and implement safety alerts.
* Document all work thoroughly in the patient management system (SystmOne).
* Support public health initiatives, including vaccination campaigns.
Person Specification
Qualifications
* Registered Pharmacist with the General Pharmaceutical Council.
* Willingness to undertake or demonstrate exemption from CPPE's Primary Care Pharmacy Programme.
* Membership of the Royal Pharmaceutical Society and the Primary Care Pharmacy Association.
Experience
* Knowledge of common acute and long-term conditions in general practice.
* Ability to collaborate with community and hospital pharmacy teams.
Skills and Knowledge
* In-depth therapeutic and clinical knowledge.
* Excellent interpersonal, influencing, and communication skills.
* Ability to communicate complex information understandably.
* Understanding of health determinants, primary care prescribing, and IT systems.
