Overview
We are seeking a Social Care Practitioner who is passionate about working within a CQC‑registered service and who shares our belief that people are best supported within their own communities to live fulfilled, family‑centred lives.
The Shared Lives Derbyshire Scheme enables and monitors arrangements for adults who meet Adult Care eligibility criteria. Our service supports mainly adults with a learning disability and/or autism to live with approved Shared Lives carers in the carers family home. We are also keen to extend this model of support to other adults who may benefit from community‑based, family‑led care.
