Living Well Assertive Outreach/ Recovery Mental Health Nurse -Adult

An exciting opportunity has arisen to recruit an experienced, Mental Health Nurse, to work in our friendly Derby City Living Well team.
The caseload itself will have complex people for you to support through their journey to recovery, as part of the long term offer of the newly established Living Well service.
This includes using a range of appropriate interventions to work with service users who have long term/ severe mental health problems and are often difficult to engage with services and treatment.

Main duties of the job
You will need a thorough knowledge of the Care Programme Approach and be able to demonstrate expertise in clinical risk assessment and management working in a community setting.
Experience of working with individuals with a forensic history and substance misuse issues would be an advantage.
We are looking for experience and commitment together with a desire to make our service the best we can.
It is essential that you have excellent communication skills and that you operate as a well-integrated member of the multi-disciplinary team maintaining a person-centred focus.You will need to have the ability to transport yourself and service users across a wide geographical area without compromising service needs.

Skills and Experience
* Community experience
* Experience in dealing with dual diagnoses/substance misuse
Skills
* Evidence of excellent verbal and non verbal communication skills
* Evidence of working/ engaging with clients with complex needs
Knowledge
* Full working knowledge of CPA and its implications
* Knowledge/ awareness of Living Well
* Evidence of risk assessment and safety planning
* Working knowledge of CTO conditions, recalling patients, Mental Health Tribunals
Qualifications
* NMC registered Mental Health Nurse
* ENB998/Mentoring in Practice or equivalent
