Job summary:

The post-holder will be responsible for a list of patients proportionate to the number of sessions worked and will deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.

Clinical responsibilities:

In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties including:

* Surgery consultations, telephone consultations, and queries
* Visiting patients at home
* Checking and signing repeat prescriptions and dealing with queries, paperwork, and correspondence in a timely fashion
* Making professional, autonomous decisions regarding presenting problems, whether self-referred or referred from other healthcare workers within the organization
* Assessing healthcare needs of patients with undifferentiated and undiagnosed problems
* Screening patients for disease risk factors and early signs of illness
* Developing care plans for health in consultation with patients and in line with current practice disease management protocols
* Providing counselling and health education
* Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
* Recording clear and contemporaneous consultation notes to agreed standards
* Collecting data for audit purposes
* Compiling and issuing computer-generated acute and repeat prescriptions, avoiding handwritten prescriptions whenever possible
* Prescribing in accordance with the practice prescribing formulary or generically when clinically appropriate

Other responsibilities within the organization:

* Awareness of and compliance with all relevant practice policies and guidelines
* A commitment to lifelong learning and audit to ensure evidence-based best practices
* Contributing to evaluation, audit, and clinical standard setting within the organization
* Contributing to the development of computer-based patient records
* Contributing to summarizing patient records and read-coding patient data
* Attending training and events organized by the practice or other agencies, where appropriate

Confidentiality:

Patients entrust us with sensitive information, which must be respected and protected. The post-holder may have access to confidential information relating to patients, carers, colleagues, and the practice itself. Such information must only be divulged to authorized persons in accordance with practice policies and procedures.

Health & safety:

The post-holder will implement and lead on health and safety and infection control measures, including the use of PPE, management of infection control procedures, safe management of sharps, and maintaining a clean and safe working environment. They will also report hazards and participate in infection control training at least twice annually.

Participation in ongoing education, training, and annual performance reviews is required to meet professional standards and support continuous improvement.

The post-holder will aim to maintain high-quality standards, assess their own performance, and contribute to team effectiveness through reflection and suggestions for improvement.

Communication:

Effective communication with team members, patients, and carers is essential, including recognizing and responding to different communication needs.

Contribution to the implementation of services:

The post-holder will apply practice policies, participate in audits, and discuss the impact of policies, standards, and guidelines on their work.
