Role
An exciting opportunity has arisen for a dynamic and innovative pharmacist to join our clinical team as the Gastroenterology Directorate Pharmacist. This post is based at the Grange University Hospital (GUH), a Specialist and Critical Care Centre, providing a centre of excellence to treat our most seriously ill patients. Excellent clinical, organisational and leadership skills are vital for this role, and you will be an integral part of the team of Medical Directorate Pharmacists at GUH.

The role requires a broad-based clinical knowledge and experience working within a specialist clinical environment. Collaborative working with the Luminal and Hepatology MDT will be a key aspect of the role, forging robust working relations to advance pharmaceutical care delivery.
Responsibilities
* Deliver specialist pharmacy services within GUH, working as part of the Medical Directorate Pharmacists team.
* Work closely with the Gastroenterology MDT, attending Luminal and Hepatology MDT meetings and ward rounds; deliver the clinical pharmacy service to this specialist area.
* Manage high-cost medicines in collaboration with the MDT, including provision of homecare medicines for the management of Inflammatory Bowel Disease and Hepatitis B & C.
* Develop medicines policies and guidelines with medical and nursing colleagues to promote evidence-based prescribing and embed national strategies into practice.
* Provide education and training to trainees, junior pharmacists and MDT colleagues, focusing on developments within Gastroenterology.
* Collaborate across all pharmacy sites to maintain continuity of care for patients and enhance working practices.
* Continue to advance clinical knowledge and skills as a Gastroenterology Specialist Pharmacist; contribute as a clinical leader within the pharmacy team.
Qualifications and Requirements
* The ability to speak Welsh is desirable; English and/or Welsh speakers are equally welcome to apply.
