The post holder will play a pivotal role in providing intensive support and treatment to patients in the community in the acute phase of a mental health crisis as an alternative to inpatient care. We are looking as a trust to change the approach as an alternative to inpatient care so that the patient can be in familiar surroundings.

Main duties of the job
This role involves intensive support and treatment to patients in the community in the acute phase of a mental health crisis as an alternative to inpatient care; monitoring patients’ safety and well-being to promote independence in the community; acting as gate keepers to admissions; supporting early discharges from hospital.
You will also be expected to work in collaboration with the Operational Team Manager, Clinical Leads/Senior Practitioners and to have close working relationships with the community teams, inpatient services, and the HTT backup across the service.

Please note that: This role requires successful candidates to be able to engage in Therapeutic Management of Violence and Aggression (TMVA) training, including the use of force techniques and the ability to attend resuscitation training and perform CPR. This is because of the high-acuity nature of the ward / community environment and the need to ensure the safety of both patients and staff. The role is conditional upon candidates being able to engage in TMVA training and resus training during the course of their employment whilst undertaking this role.

Detailed job description and main responsibilities
Clinical duties
To triage referrals and allocate work to junior staff as appropriate.
To provide a comprehensive mental health assessment for service users with complex needs in the acute phase of a mental health crisis.
To assess current risks and identify how the risk and service users’ mental health needs would best be met.
To formulate, implement and review individualised Recovery Care Plans with patients and their carers that aims to address their psychological, physical & social needs.
To build and maintain therapeutic relationships with service users and carers and involve them in drawing up care plans.
To work with service users’ care co-ordinator/ lead professional as appropriate to ensure continuity of care.
To provide information and advice to carers to support them through the crisis period.
To administer, supervise and monitor prescribed medication advising service users and carers on storage, administration and possible side effects.
