St Lukes Hospice is looking for an Integrated Care Facilitator, who has experience working within health and social care, who has a passion to use this knowledge to enhance the lives of our Palliative care patients and their significant others.
About the role
As their Integrated Care Facilitator, you will use your specialist knowledge of the health and social care system to support your clinical colleagues within the Hospice, primary care and wider healthcare providers. You will be working directly with patients and their significant others in the communities where they live, helping to develop a deep understanding of the many local initiatives which can support them in a more creative way.
You will be pivotal in helping us develop and evaluate this exciting role, and your views and experience will be critical in helping us be the very best at what we do. We recognise the critical impact and challenges current health and social care provision has on our patients and their significant others, and we are looking for someone who can maximise the support and choice for our patients, from referral through to end of life.
We are looking for someone who uses their own imitative and is able to work as part of a team, who can provide support across our 3 community hubs.
About you
It is your values that are most important to us. Experience of working within the clinical setting and/or specialist palliative care would be an advantage but not essential and training will be given to the right individuals. Knowledge of care provision across Plymouth and South Hams and West Devon would be a particular advantage.
