About the Role
Your focus as a Team Leader will be to support the home management team in delivering high-quality care by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.
Other responsibilities include:
1. Leading the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for executing those plans.
2. Assisting in the smooth transition of discharges from the home, including documentation, medication management, and keeping residents' next of kin fully informed where appropriate.
3. Monitoring the well-being of each resident, being aware of any changes impacting care, and ensuring that care plans are updated accordingly.
4. Practicing safe systems of work across various tasks, including moving and handling of residents, by assessing risks and ensuring safety for residents, visitors, staff, and oneself.
About You

Our ideal candidate will:
* Hold an NVQ Level 3 or equivalent in care for care homes.
* Have at least two years’ experience in a senior role working with residents.
* Possess effective verbal and written communication skills.
* Demonstrate leadership ability and responsibility in the absence of the manager.
