We are seeking a Practice Nurse to join our small friendly surgery. The successful applicant must be well motivated and flexible as our Practice only employs one Practice Nurse and a Health Care Assistant. Confident and competent in reviewing patients is essential.

A good knowledge of QOF - Chronic disease management is essential. The applicant will need to work autonomously.

About us

We are a small family practice based in Bolton. We are afriendly team and pride ourselves on providing high quality patient care.

We have a list size of 4100 patients, have a supportive teamof GP's and ANP. We have full access to a range of ARRS staff including MSK andPharmacists.

The clinicians are supported by a Practice Manager, Assistant Practice Manager, 1 Secretary, 5 Receptionists and 1 Health Care Assistant.

We use EMIS clinical system.


Job responsibilities

JobSummary:

The post holder is an experienced nurse, who actingwithin their professional boundaries will provide care for the patientpresenting with long term conditions and raise awareness. The nurse isresponsible for the care delivered, demonstrating clinical decision makingskills in the management of patients. They will work collaboratively within thepractice team to meet the needs of patients, support the delivery of policy andprocedure, and provide nurse leadership as required. They will also be a part ofsupporting and mentoring the practice healthcare assistant.

JobResponsibilities:

* Assess, plan, develop, implement and evaluateprogrammes which promote health and wellbeing, and prevent adverse effects onhealth and well being
* Assess, plan, develop, implement and evaluateindividual treatment plans for patient with a known long term condition
* Proactively identify, diagnose and manage treatmentplans for patients at risk of developing a long term condition as appropriate.
* Work with other health care professionals todiagnose, monitor, manage and treat long term conditions.
* Bloodpressure, pulse rate and rhythm, temperature, height and weight -body massindex, venepuncture, glucose tolerancetest, peak flow readings, inhaler technique, cytology, travel and childhoodvaccinations, chronic disease management
* Prioritise health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiating effective emergency care.
* Supports patients to adopt health promotionstrategies that encourage patients to live healthily, and apply principles ofself-care
* Deliver opportunistic health promotion usingopportunities such as new patient medicals
* Provide information and advice on prescribed orover the counter medications, on medication regimens, side effects andinteractions
* Support and guide women attending for familyplanning advice and manage the needs of women attending for cervical cytology
* Implement and participate in immunisationprogrammes for both adults and children
* Advise, support and where appropriate vaccinatepatients travelling abroad
* Undertakethe collection, storage and despatch of pathological specimens includingintravenous blood samples, swabs, smears etc. and ensure the cleanliness of anyequipment used. Perform anyinvestigatory procedures requested by the GPs
* Supportingthe practice with the Quality Outcome Framework (QOF), Enhanced Service and BoltonQuality Contract (BQC)
* Workwith the practice manager in the organisation and implementation of call andrecall of patients for screening/health promotion, immunisation and diseasemanagement clinics, as necessary to meet the agreed needs of the service.
* Towork collaboratively with the Healthcare Assistant and others as appropriate,providing professional support as necessary to improve and develop practice andcompetence to ensure fitness for practice and assure service quality.
* Maintaingeneral tidiness and cleanliness of nurses and clinical rooms, inventory ofequipment and fit for purpose record
* Overseeclinical stock systems to ensure safe and sufficient supplies.
* Ensureregular checks of clinical consumables, emergency equipment, sharps andclinical supplies
* Ensureexpiry dates are monitored and acted upon in a timely manner
* Overseeand support the HCA in ordering clinical and lab stock orders
* Provideclinical oversight to HCAs or admin staff undertaking delegated stock checks
* Escalatestock shortages or risks promptly
* Takeresponsibility for oversight of lab stock ordering including blood bottles,swabs sample bags and forms and cytology equipment
* Monitorstock usage and maintain appropriate levels, removed expired or damaged stock.
* Liaisewith laboratories and Practice Manager regarding supply issues.
* Takelead responsibility for vaccine management and cold chain compliance
* Ensurevaccines are store correctly according to UKHSA guidance.
* Organised,labelled and rotated appropriately
* Checkedroutinely for expiry dates
* Monitorvaccine stock levels to prevent wastage or shortages
* Investigateand document cold chain breaches where documented
* Onoccasion there may be a requirement to undertake home visit
* Anyother delegated duties appropriate to the post

* Recognise and work withinown competence and professional code of conduct as regulated by the Nursing andMidwifery Council (NMC)
* Produce accurate andcomplete records of patient consultation consistent with legislation, policiesand procedures
* Prioritise, organise andmanage own workload in a manner that maintains and promotes quality
* Participate in themaintenance of quality governance systems and processes across the organisation
* Evaluate the patientsresponse to health care provision and the effectiveness of care
* Support and participatein shared learning across the practice and the wider organisation

* Participate in theperformance monitoring of the team, providing feedback as appropriate
* Understand and applylegal policy the supports the identification of vulnerable and abused childrenand adults, being aware of the statutory health procedure and local guidance
* Work within policiesrelating to domestic violence, vulnerable persons, substance and alcoholmisuse; and refer as appropriate
* Attend training /informative events onbehalf of the nursing team/practice as and when required

* Ensure safe storage,rotation and disposal of vaccines and drugs is undertaken. Oversee themonitoring, stock control and documentation immunisations clinical stock, PPEand any other items as that are appropriate

Person Specification


Experience

* Experience of working in a Primary Care Setting and experience of chronic disease management. Must be able to work autonomously.
* Nurse prescriber.
