An exciting opportunity has arisen for a full time experienced band 6 Physiotherapist with expertise in Intermediate Care, Rehabilitation, Rapid Response, and Admission Avoidance. We are seeking a well-organised individual who is passionate about promoting independence and providing physiotherapy interventions primarily within our Intermediate Care Units and within a person's own home, utilising a home-first approach.
Role
We are thrilled to be able to welcome you to our friendly team and while the main focus of the role is within the IMC units and home-based IMC, you will also have the opportunity to collaborate across various services to ensure a comprehensive approach to patient care. This will include:
* Killelea House (Enhanced and Residential IMC)
* Home-based Intermediate Care and Reablement
* Elmhurst Short Stay Intermediate Care Service
* Bury Rapid Community Response & Hospital at Home Service
You will be part of a multidisciplinary team that includes Physiotherapists, Occupational Therapists, Advanced Practitioners, Nurses, Social Workers, Care Support Workers, and Administrators. Our service operates 7 days a week, covering the hours of 8 am to 8 pm, including weekends. Therefore, the ability to work shifts within these hours is essential.
We are particularly interested in hearing from candidates with significant NHS experience, a relevant degree, HCPC registration, and a proven track record of working with a multidisciplinary team for this role.
The Intermediate Tier prides itself on providing strengths-based assessments and interventions to the people of Bury, helping them regain or attain as much independence as possible in their homes.
Our patients are at the heart of everything we do! We are looking for someone committed to being part of a healthcare team that delivers excellent care and support to our patients. To help us achieve this, you will be responsible for your own caseload, performing patient therapy assessments, providing a diagnosis, developing, implementing, and evaluating individualised holistic treatment programmes, and delivering a high standard of evidence-based care.
Within all our Intermediate Tier services, you will work within and have support from a multi-disciplinary team, follow appropriate pathways of care, and liaise with and refer to other agencies as appropriate.
We seek someone hardworking and motivated with excellent clinical, communication, organisational, and time management skills. A flexible approach to day-to-day tasks is a must to meet the demands of the service. You will also need to travel across the service footprint when necessary, in line with service needs, so a UK driving license and access to vehicle or a means of mobility to travel across the service footprint is essential.
