Early Help Team Social Worker - Permanent £32,000 - £42,000 The Early Help Children's Team works with children, young people, and their families at the earliest signs of need, aiming to prevent issues from escalating and reduce the need for more intensive statutory interventions. We support families to build on their strengths and achieve positive outcomes. Duties: Conducting Early Help Assessments (EHAs) to identify the needs of children and families, working collaboratively with them to develop and implement Early Help Plans. Providing direct support and interventions to families, which may include parenting advice, signposting to community resources, and facilitating access to specialist services. Working proactively with families to address a range of challenges such as domestic abuse, parental mental health, substance misuse, and school attendance issues. Collaborating effectively with a wide range of partners, including schools, health visitors, GPs, housing services, and voluntary sector organisations, to ensure a coordinated and holistic approach to family support. Building strong, trusting relationships with children and families, ensuring their voices are heard and their views inform the support they receive. Monitoring and reviewing the progress of Early Help Plans, adapting interventions as needed to ensure they remain effective and responsive to changing family circumstances. Promoting resilience within families and empowering them to develop their own solutions and support networks. Maintaining accurate and up-to-date records of assessments, plans, and interventions, adhering to all relevant policies and procedures.
