Job Summary
The Health Visitor (SCPHN) leads a defined caseload and delivers the Healthy Child Programme and national high‑impact areas within a progressive universal model.
Key Responsibilities
* Conduct advanced assessment, clinical decision‑making and leadership to identify need, manage risk and deliver targeted interventions to children and families.
* Co‑ordinate care across a skill‑mixed team, providing supervision, delegation and support to ensure safe and effective service delivery.
* Partner with multi‑agency colleagues to safeguard, promote early intervention, support health promotion and drive service development.
* Manage complex workloads, maintain accurate records, evaluate outcomes and use data and evidence to inform practice and continuous improvement.
* Lead the delivery of the Health Visiting Delivery Model and Healthy Child Programme for an identified caseload, ensuring commissioning outcomes are met and health inequalities reduced.
* Assess, plan, implement and evaluate programmes of care with a strong focus on early identification of need, safeguarding and improving outcomes for children, especially within the first 1001 days.
* Collaborate with families, communities and multi‑agency partners to ensure effective, coordinated, high‑quality service delivery.
Qualifications and Requirements
Registered Health Visitor (SCPF/SCNH) with current registration and competency.
Must hold a valid UK right to work and be able to maintain it throughout employment.
Registered healthcare practitioner roles only supported for sponsorship – applicants who need a visa will be considered only if they meet trust eligibility criteria.
