We are seeking a full or part-time Prescribing Pharmacist to work until 20:00 one day per week. Completion of CPPE is desirable.

Main duties of the job
* Repeat prescribing
* Conducting face-to-face and telephone appointments with patients
* Managing chronic diseases
* Reconciling medicines post-hospital discharge
* Providing telephone support to patients
* Handling care home prescriptions
* Performing clinical audits
* Supporting the prescribing lead in maximizing points in the Prescribing Incentive Scheme
* Ensuring patient safety through drug safety alerts and CQC compliance
* Training students
* Managing secondary care prescriptions
* Liaising with local community pharmacists

Job responsibilities
* Manage the repeat prescribing authorization process, review patient requests, and make necessary changes as an Independent Prescriber
* Ensure patients undergo necessary monitoring tests
* Switch to more cost-effective medication options where appropriate
* Promote and increase uptake of Electronic Prescriptions
* Synchronize repeat prescriptions as needed
* Link prescriptions to medical conditions
* Reconcile medicines after hospital discharge, rectify unexplained changes, and perform medication reviews
* Coordinate follow-up tests and collaborate with patients and community pharmacists for post-discharge medication management
* Provide urgent medication prescriptions and reviews via telephone
* Support community pharmacists and nurses with prescription queries
* Review and issue prescriptions for care home patients and liaise with Care home pharmacists
* Perform clinical audits to support the Prescribing Incentive Scheme
* Assist in achieving maximum points in the Prescribing Incentive Scheme and ensure compliance with safety alerts and CQC standards
* Provide training to NHS students or trainees
* Manage requests for new medications from secondary care outpatient appointments, including patient education on medication use and monitoring
* Facilitate communication with local pharmacies to optimize service delivery
* Conduct face-to-face medication reviews and manage minor illness and chronic disease appointments based on experience/training
Person Specification
Qualifications
* Registration as a pharmacist with the General Pharmaceutical Council
* Full driving license and access to a vehicle
* Membership of the Royal Pharmaceutical Society
* Independent prescribing qualification or willingness to undertake it
* Experience working within a multidisciplinary primary care team
* Proactive, IT literate, and able to teach and problem-solve effectively
* Strong clinical skills and decision-making abilities
* Patient-centered approach and team-oriented mindset

This role requires a DBS check due to the Rehabilitation of Offenders Act 1975 (Exceptions Order 1975).
Starting salary ranges from £23.00 to £28.86 per hour, depending on experience.
