Overview

Our Acute Frailty team are seeking a highly motivated practitioner with a passion to improve the experience of our ageing and frail population. The post holder must have knowledge and prior experience of managing patients with high clinical frailty scores and undifferentiated complex care presentations. Excellent communication skills are essential to communicate with patients and carers regarding clinical conditions, and experience in sensitive discussions around anticipatory care planning would be advantageous. The role may involve working across organisational boundaries and in partnership with other health and social care teams.

The successful candidate will join a team that utilises Comprehensive Geriatric Assessment as a method of screening frailty and planning multidisciplinary care. In addition to initial screening, the successful candidate will prioritise care and provide daily ongoing medical review of frail patients alongside the wider multidisciplinary team. You must be able to demonstrate good organisational skills, excellent communication skills and a commitment to continuing professional development. Shift pattern is variable depending on location.

We may also consider applications for trainee posts and will support the development of individuals to meet advanced practice competencies using a triangulation approach of academic courses, professional supervision and clinical practice supervision. Applicants for trainee posts should have demonstrated evidence of undertaking courses relevant to the advanced practice pathway.

This post will also be considered as in training position under Annex 21 of the Agenda for Change terms & conditions.
