Job Title: Qualified Social Worker – Reablement Assessment and Bridging Team
Location: Ealing
Rate: £35.00
What will your responsibilities be?
You will assess clients’ needs, capabilities, and goals, then develop personalized reablement plans focused on improving independence. You will provide direct support and coaching to clients, teaching new skills and promoting self-care to help achieve reablement goals. Additionally, you will monitor progress, adjust plans as needed, and collaborate with healthcare professionals and community services to ensure holistic support and advocacy for clients.

Qualifications and Experiences:
To be eligible for this role, you must have Social Work England registration and be eligible to work in the UK. Additionally, you need to hold a full UK driving licence and possess post-qualified experience. A degree or equivalent qualification in Social Work is also required.
