Children Community Learning Disability Nurse

We are seeking 2 highly motivated Registered Learning Disability Nurses to join the Children and Young People’s Learning Disability Service in Surrey. This Surrey-wide service sits across 4 bases: Redhill, Leatherhead, Godalming & Chertsey. CLD is a nurse-led service with Psychology / APs / Psychiatry / Music Therapy / Admin Support. CLD works collaboratively with families, carers and the young person to establish a plan of care. The majority of needs presenting are behaviour and anxiety related. CLD has a Behaviour Pathway based on PBS which also incorporates Behaviour Clinics run termly with the nurses and APs. CLD links with education, school nursing, social care, Paediatrics and other agencies supporting the young person. CLD holds 2 Clinical Issues meetings each month, along with a monthly Business Meeting and quarterly LD Forum. Regular supervision and appraisal with opportunities for career development are offered. You will need to be a car driver and have access to a car for work; the role will take you across a geographical area of Surrey visiting family homes, schools and other locations. You will also be required to attend training and meetings at Leatherhead HQ. You will need to be a Registered Learning Disability Nurse with knowledge of PBS. Ideally you will have experience of working with children; however skills are transferable between adults and children. CLD follows the I Thrive Model. You will hold a caseload of children and young people in the community, following a Discovery Conversation where the presenting need has been identified. Demonstrate expertise and specialist nursing skills in areas associated with learning disability such as crisis intervention, alternative communication, assessment and management of people with additional mental health needs, epilepsy, medication management, challenging behaviour and other conditions. To actively involve people in developing and agreeing their own care plan, to act as an advocate and ensure proper regard for dignity, choice, self-esteem, racial, cultural, sexual and religious needs and preferences. To carry out comprehensive assessments, plan, implement and evaluate delivery of nursing interventions as part of a holistic approach working in a variety of settings. To practice within the Positive Behaviour Support framework. Responsibilities and Qualifications

* Hold a caseload of children and young people in the community with a focus on learning disability care planning and delivery.
* Demonstrate expertise in crisis intervention, communication supports, management of co-occurring mental health needs, epilepsy, medication management and challenging behaviour.
* Work within a Positive Behaviour Support framework and participate in multi-disciplinary teams.
* Collaborate with families, carers and the young person to develop and agree their own care plan; advocate for dignity, choice, and cultural needs.
* Carry out comprehensive assessments; plan, implement and evaluate nursing interventions in various settings.
* Attend training and meetings (Leatherhead HQ) and participate in regular supervision and appraisal with opportunities for career development.
* Be a Registered Learning Disability Nurse with knowledge of PBS; driving licence and access to a car required.
