Occupational Therapist – Band 6 Community OT / Care Coordinator
Redbridge Mental Health and Wellness Team (MHWT) – South, part of NHS North East London
Overview
This role is based in the Redbridge District, within the NELFT system. The post holder will provide specialist functional assessments, care coordination and group interventions as part of an integrated multidisciplinary team. The full‑time position offers a protected OT workload split 50/50 between OT assessments and care coordination duties.
Key Responsibilities
* Conduct functional assessments of daily living skills using the Model of Human Occupational (MHO) assessment tools.
* Implement community care plans, provide adaptive equipment, and maintain high service‑user care standards.
* Lead and support group programmes alongside OT staff, support‑time recovery workers and peer support workers.
* Collaborate with IPS employment workers to support service users’ employment goals.
* Provide comprehensive mental health assessments, key‑work, and co‑ordinate care in line with Care Programme Approach (CPA) policy.
* Implement and review care plans, ensuring regular reviews and evaluation of outcomes.
* Engage carers and families in care planning and support.
* Ensure safeguarding of children and adults in accordance with the Care Act 2014 and local safeguarding policies.
Employment and Onboarding
All new starters join on the first Monday of each month and undertake a comprehensive induction lasting up to two weeks, including mandatory training, systems training and equipment allocation. The role is subject to a probationary period unless you are an internal applicant exempt from probation.
Remuneration
High‑cost area supplement of 15% of the basic salary (pro‑rata for part‑time staff, minimum £4,714, maximum £5,941).
Location & Working Hours
Monday to Friday, 9‑am to 5‑pm. Some flexibility may be required; car driver vehicle availability is desirable but not essential.
